When someone dies in Nebraska

Certified copies come from the Nebraska Department of Health and Human Services, Vital Records (official site).

A certified copy by mail is $16, paid by check or money order made out to Vital Records, because credit cards are not accepted by mail. Send it to Nebraska Vital Records, P.O. Box 95065, Lincoln, NE 68509-5065 with a photocopy of your government issued photo ID. Your application needs the full name of the person who died including a married name or any other names used, the city or town of death, a span of years if you do not know the exact date, how you are related, and what the record will be used for.
