Arkansas is one of the cheaper states: $10 for the first copy and $8 for each additional copy ordered at the same time. Walking in is the fastest option, and most requests are filled the SAME DAY if you arrive by 4:00pm at 4815 West Markham St, Little Rock, AR 72205, open weekdays 8:00am to 4:30pm. Mail takes 10 to 14 days plus delivery, with a check or money order to Arkansas Department of Health, no cash or temporary checks. Phone orders are toll free at 866-209-9482.
